  15. Right to plan of care ,progress & information on their health care needs.

 

PATIENT RESPONSIBILITIES:

  1. To provide complete & accurate information including patient’s full name ,address, contact number, age, insurance details and other information as required.
  2. To provide accurate information about patient’s past & present health ,including past illness or health problems ,hospitalizations ,allergies and the current or past use of medication.
  3. To read and understand all medical forms including consent forms thoroughly prior to signing.
  4. Participate and understand your treatment plan and your role in that plan.
  5. Making informed decisions about your care.
  6. Refrain from violent behaviour and any verbal abuse with the staff.
  7. Consideration for the rights and respect for the property of the other patients and staff.
  8. Assistance in hospital efforts to limit noise.
  9. Assistance in hospital efforts to limit the number of visitors for safety and noise control.
  10. Compliance with smoke –free environment policy.
  11. To comply with hospital rules and regulations from time to time.
  12. Providing complete accurate and timely information about the sources of payment for care provided by the hospital.
  13. Fulfilling financial obligations of health as promptly as possible.
  14. To keep any appointments made, or notify the hospital or clinic as early as possible in case you are not able to do so.
